Cap broadly umbonate, 9 cm, yellow at the edges turning yellow-orange and then darkening to a brownish yellow at the center. Deeply striate over about 1/3 of cap. One white volval patch on the cap. Cap flesh cream, 2 mm thick where stipe meets the cap, thinning to almost nothing toward the edge. Gills free, close, pale yellow, 5 mm deep at most. Stipe pale yellow, fading to cream at the base, somewhat fibrous. 9 cm long by 9 mm thick at apex and 12 mm at base, with pale yellow annulus. Base surrounded by a robust cream-colored volva, which is attached only at the base of the stipe. Stipe flesh pale yellow, stuffed white. Growing singly under mixed oak, birch, white pine, hemlock.
